This position provides supportive case management for Safe Haven Program participants. Assesses service needs, makes and follow ups on referrals, provides support and advocacy.SMOC is a private non-profit multi-service agency that provides a comprehensive array of services including: Behavioral Health Services, Comprehensive Housing Services, Employment and Education, and Family & Nutrition. SOME RESPONSIBILITIES INCLUDE
• Assist clients in finding and maintaining appropriate housing.
• Provide case management, perform intakes/assessments, make referrals and advocate for Safe Haven Program Participants.
• Maintain and document weekly individual meetings with all program participants.
• Assist clients with basic living skills, social skills, money management and other issues as needed.
• Work with the Opportunity Center as part of that team.
• Attend weekly staff meetings and other meetings as necessary.
• Maintain client files including weekly case notes and all relevant data. Ensure accuracy and timeliness.
• Assist with special projects and additional assignments as needed.
• Keep accurate detailed records in accordance with the billing requirements.
• Provide on call coverage.
KNOWLEDGE AND SKILL REQUIREMENTS
• Well-organized person with excellent verbal and written skills
• Ability to initiate task independently as well as carry out assigned duties
• Ability to work in a very busy environment and multi task
• Experience with homeless population
• Must have a valid driver’s license, reliable transportation and ability to meet insurance standards
PHYSICAL REQUIREMENTS
• Ability to drive.
• Ability to ascend/descend stairs
WORKING CONDITIONS
The Safe Haven Case Manager will spend time both on site at the Safe Haven Program and at the Opportunity Center as part of the Opportunity Center Team. The Case Manager will also be responsible for transporting clients to various appointments, medical or otherwise. The Case Manager will also be required to provide on-call after hours coverage as needed. As part of the responsibilities of this position, the Case Manager will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services.
